Insurance Questions

I do not take insurance at this time. This allows the ability to customize your care exactly to your needs without the intervention and restrictions of an insurance company.

 

If requested, I am able to provide superbills for you to submit to your insurance company. It is important to note that superbills require a diagnosis and billing codes that will be included in your medical records. Approval for reimbursement is not guaranteed so I strongly encourage you to contact your insurance company to confirm and discuss your out-of-network benefits, as it is your responsibility to be aware of your coverage.​

Below please find some questions to help guide your conversation with your insurance company. Please note this is not an exhaustive list.

  • Do I have out-of-network mental health benefits?

  • Does my plan include coverage for mental health services provided by an Associate-level clinician? (They should tell you based on my NPI number, 1104609395, but it is always good to confirm)

  • ​Is there a deductible for out-of-network services? If so, how much is it?

​

  • Is there a maximum for out-of-pocket costs for out-of-network services? If so, how much is it?

​

  • What is the reimbursement percentage for costs?

​

  • How do I submit claims for out-of-network services and what documentation is necessary?

  • How long does it generally take to receive reimbursement?
